Independent social work support to develop clear, person-centred support plans that reflect individual needs, wishes, strengths and desired outcomes. We can complete or review support plans, identify eligible care and support needs, complete independent Care act assessments, assess risks, and make practical recommendations about the support required.

We work collaboratively with individuals, families, care providers and professionals to ensure support is appropriate, proportionate and focused on promoting independence, choice and wellbeing..

We create personalised social support packages based around the individual’s interests, goals and preferred activities. This can include researching local groups, clubs, activities and community resources, making connections with suitable services, and developing a clear package of support to increase social interaction, independence and community participation.

We can also help identify and coordinate directly purchased or commissioned support where a more individualised approach is needed. This includes sourcing for direct payment packages.

I provide independent, decision-specific Mental Capacity Assessments in accordance with the Mental Capacity Act 2005. Assessments are completed thoroughly and sensitively, with clear consideration of the individual’s ability to understand, retain, use or weigh relevant information and communicate their decision.

Where required, I can provide a detailed written report setting out my assessment, evidence and professional conclusions.

Our Court of Protection services offer expert guidance through legal processes to protect vulnerable individuals. We help clients navigate the complexities of court requirements and ensure their rights are upheld.

We provide assessments and advice regarding Deprivation of Liberty Safeguards to ensure that individuals are treated lawfully and with dignity. Our approach facilitates collaboration with care teams to support individuals effectively in line with the new AGNI ruling.

Our immigration assessment services cater to vulnerable migrants requiring support in navigating legal processes. We ensure comprehensive documentation and expert guidance throughout the journey.

I provide practical support for individuals who may be struggling to manage their home environment due to hoarding, self-neglect, reduced mobility, mental health difficulties or changes in circumstances.

Support can include assessing what is required, arranging or coordinating deep cleaning and decluttering services, working alongside the individual at their own pace, and helping to put longer-term support in place to maintain a safe and manageable home.

Where appropriate, I can also liaise with families, care providers, housing services and other professionals to ensure the support is person-centred, respectful and sustainable..

I provide independent advice and practical support to individuals and families navigating adult social care. This can include guidance around Lasting Powers of Attorney (LPA), Direct Payments, care and support options, and accessing appropriate services.

I can advocate on your behalf with local authorities and other professionals, attend meetings where required, and prepare independent social work assessments and professional reports to clearly evidence an individual’s care and support needs.

Reports can be used to support requests for Care Act assessments, increased care packages, Direct Payments, reviews of existing support, community-based services or other appropriate provision.
